PRE-EMPLOYMENT STATEMENT (Read Carefully)

I certify that all information supplied in this application, and any attatched resume, is true and correct. I understand that, because City Bank & Trust Company ("the Company") will rely on this application in making its employment decision, any false or misleading information furnished by me regarding this application or termination of employment by the Company.  In addition, I understand that failure to answer truthfully any of the above questions may result in a denial of any right I or my dependents may have to workers' compensation benefits, medical treatment and expeneses. LSM-RS23:1208.1

In consideration of my employment, I agree to conform to the rules and regulations of the Company, and further agree that my employment and compensation are at the will of the Company and can be terminated, with or without cause, and with or without notice, at any time at the option of either the Company or myself. I understand and agree that these terms can only be modified by the President and Chief Executive Officer of the Company in writing, provided that such writing specifically acknowledges that it is a modification of this agreement and is signed by the President and Chief Executive Officer. No supervisor, representative, agent, or employee of the Company has now or has had in the past any authority to enter into any agreement for employment for a specified period of  time, or to make any agreement which is contrary to or a modification of the above terms, nor can any policies of the Company either written or oral, modify the above terms.

• I understand that a drug test and a negative result therefrom may be required as conditions of employment.
• I understand that I may be required to have a physical examination (which is paid for by the Bank and conducted by a doctor selected by the Bank.
• I hereby agree to being fingerprinted before or during my employment and agree to my fingerprint record being processed by the FBI.
• I authorize investigation of all statements contained in this application. I understand that misrepresentation or omission of facts called for is cause for dismissal. Further, I understand and agree that my employment is for no definite period and may, regardless of the date of payment of my wages and salary, be terminated at any time without previous notice.
• I fully understand because of the nature of the business conducted by the Bank that all information, whether written, spoken or obtained, and all files and records of any and every description relating to the business of the Bank or to anyone with whom the Bank has dealing, constitute privileged matters and are to be treated in a strictly confidential manner. I fully understand and agree that, should I enter the employ of the Bank, I am not to, and will not at anytime communicate or reveal any business personnel within the Bank or to anyone outside the Bank. I also understand that any violation of the foregoing shall be sufficient grounds for termination of my employment.
• In the event I am employed by the Bank, I will comply with all rules and regulations as set forth in the bank's policy manual or other communications distributed to all employees.
• I certify that all statements made by me on this application are true and complete to the best of my knowledge and that I have withheld nothing that would, if disclosed, affect this application unfavorably.
• I understand that in the event of suspected embezzlement, theft or misappropriations, you will use polygraph testing in compliance with the Employee Protections Act.
• I understand that if I am employed, a photograph may be later required for attatchment to my personnel file.
• I hereby acknowledge that I have read the above statements and understand the same.

I hereby authorize all educational institutions which I have attended, all branches of U.S. military service in which I have served, all of my former employers, all credit bureaus, all court systems, and all of their representatives to furnish to City Bank & Trust Company or its representatives any and all information concerning my education, military service former employment, credit history, and/or criminal convictions, In addition, I hereby agree to hold harmless and to release all said institutions, services, employers, bureaus, courts, and representatives from any and all claims that I may have, or which may arise, against any and/or all of them, including City Bank & Trust Company, as a result of their furnishing informaiton to City Bank & Trust Company
